Home Delivery Information

We were unable to obtain prescription-refill records from your previous home-delivery provider, but by following the steps below, you can continue enjoying the convenience of prescriptions delivered right to your door.

1. Ask your doctor to write a new prescription, plus refills for up to one year, if appropriate.

2. Complete and sign a New Patient Home Delivery Form.

3. Put your new prescription and payment into the completed form, and mail it no earlier than December 23, 2008.

4. Your order will be mailed directly to your home. With each order, you'll receive a form showing the date when you can refill your prescription.

Your doctor can also fax your prescriptions directly to Express Scripts at 1-800-613-5628. Please allow enough time to process your order so you don't run out of medication. Your order should arrive within 10 to 14 days from when it's received.
