GlobalHealth health plans are available to person’s who live or work one of the following service areas: Blaine, Canadian, Cleveland, Creek, Garvin, Grady, Harmon, Hughes, Johnston, Lincoln, Logan, Major, Mayes, McClain, Oklahoma, Osage, Okmulgee, Pontotoc, Pottawatomie, Rogers, Seminole, Tulsa, Wagoner. You must also meet the eligibility requirements defined by your employer’s group contract with GlobalHealth. Since your employer contracts for your healthcare benefits, you are the subscriber to the health plan. Your spouse and children are dependents.
Enrollment Limitations
Members may enroll only during specific times of the year and/or under certain conditions. Contact GlobalHealth at 1-405-280-5600 (TTY for the hearing impaired: 1-800-522-8506) 8 AM to 5 PM, 5 days a week to obtain more information.
