Disease Management
Focuses on assisting or supporting you and/or your caregivers to maintain your health. The Disease Management staff will provide a program that includes, but not limited to:

Scheduled follow up calls

Facilitate physician visits where necessary

Provide education either mailed or telephonic
The website is available to use and contains material from recognized agencies such as the American Heart Association, American Diabetic Association as well as a library containing written, picture and video of many procedures, medications. You can contact the staff during regular business hours or the staff will contact you to explain how disease management can provide support.
Case management
Supports and assists you in many ways. For instance, a Case Manager may contact you after you have been discharged from a hospital stay to ensure you have the services you need for an uneventful recovery or check with you on any new medications, new diagnosis and any education you may want or need.
